Doing the male fertility testing up-front at the start of your fertility journey can save you from the emotional and financial impact of proceeding blindly with assisted reproductive technology ART attempts at pregnancy without knowing the fertility status of the male.

The Male Reproductive Health Panel (MRHP) was developed by David B. Brown who also determined the performance characteristics. The MRHP has not been cleared or approved by the US Food and Drug Administration (FDA). FDA does not require these tests to go through premarket FDA review. This panel of tests is used for clinical purposes. It should not be regarded as investigational or for research. The MRHP has been performed commercially for over 10 years by Male Fertility Diagnostics, LLC in partnership with UniLab of Dade, Inc.

Both laboratories are certified under the Clinical Laboratory Improvement Amendments of 1988 (CLIA) as qualified to perform high complexity clinical laboratory testing. *United States Patent No.: US 9,157,909 B2: “Male Reproductive Health Panel and Uses Thereof.” Issued: October 13, 2015. SDAD and SDD Services

Two important tests in the context of sperm DNA transformation are the Sperm DNA Decondensation Test (SDD) and Sperm DNA Accelerated Decondensation (SDAD).

After a sperm fertilizes an egg, its DNA undergoes a crucial transformation to effectively merge with the egg's DNA. This initial phase is referred to as decondensation.

These tests evaluate whether the sperm DNA undergoes the typical decondensation process.

If this process fails to occur as expected, it can result in a partially or completely unreadable DNA message.

Such an outcome increases the likelihood of early pregnancy loss or may even prevent pregnancy altogether. Therefore, these assessments play a crucial role in understanding and addressing potential challenges in the fertilization process.

Frequently Asked Questions

Q:

Why should infertile couples consider having the Male Reproductive Health Panel (MRHP) of tests?

Approximately 50% of infertile couples have male factor issues; 40% predominantly male, or 10% partially attributed to the male partner. The female accounts for the 50% of couples with female factor issues; 40% predominately female, or 10% partially attributed to the female partner. The MRHP of tests identifies approximately

Despite knowing this, most fertility doctors initially focus only on the female. Most fertility specialists only perform a standard semen analysis of:

-Sperm count

-Strict Morphology

-Sperm Motility

Standard semen analysis provides little information on a sperm’s capability to deliver DNA into the egg or on the quality of the DNA in the fertilized egg, all important to facilitating a pregnancy that will result in a live birth.

We provide a sperm count, but do not provide morphology or motility scores, as these test scores provide little information for the fertility doctor to determine your treatment options.

Q:

Why is it important to know at the start of an infertile couple’s work-up if there are male factor issues that need to be addressed?

Knowing this at the start will help the fertility specialists provide you with individualized ‘best’ treatment algorithms increasing your chances for taking home a baby sooner while minimizing your emotional and financial impact.

Also, the male has a complete turnover of sperm every 8 weeks. If you can identify male factor issues early on, you can potentially correct the problem resulting in a live birth without the need for expensive Assisted Reproductive Technology (ART) attempts at pregnancy.

Q:

What is the Sperm DNA Decondensation (SDD) Test?

The SDD Test is a sperm function test that measures delayed sperm DNA decondensation rates. The SDD test assesses if the sperm DNA will function properly after entering the egg during the fertilization process. Once in the egg, the sperm’s DNA goes through a series of steps to combine with the egg’s DNA that includes sperm nuclear swelling. This event must occur correctly to have a successful fertilization that will result in a live birth.

If there is a delay in sperm nuclear swelling, such sperm will not be useful for IUI or IVF attempts at pregnancy. Such individuals should be fast tracked to intracytoplasmic sperm injection (ICSI) attempts at pregnancy where they will have successful attempts at pregnancy consistent with the clinic’s overall ICSI live birth rates.

Q:

What is the Sperm Chromatin Fragmentation Assay (SCFA)?

The SCFA is a sperm DNA integrity test that identifies individuals who produce sperm with poor DNA integrity. Specifically, patients with DNA Fragmentation Ind. Such individuals will have a low chance of a live birth with natural conception or Intrauterine Insemination (IUI) and should be fast tracked to in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) attempts at pregnancy where they will have successful attempts at pregnancy consistent with the clinic’s overall IVF or ICSI live birth rates.

Q:

What is the cost of the Male Reproductive Health Panel and what does this include?

The cost is $575 which includes the sample kit delivered to your address and the health panel results.

Q:

When should the male be tested for male factor issues?

The MRHP should be performed for any couple who has been involuntarily infertile for 1 or more years. This can be done prior to seeing a fertility specialist. Males identified as having male factor issues will be advised to see a fertility specialist.

We will provide you with a report of your MRHP test scores that you will bring to the fertility specialist who will use these results to help determine your best treatment options early on in the process.

Note: in the case where you are found to be normal in the MRHP, you may opt to continue trying to conceive naturally. If no successful pregnancies occur, you should see a fertility specialist who can then focus on the female.

Q:

What tests make up the Male Reproductive Health Panel (MRHP)?

The MRHP is a group of 3 diagnostic tests used to assess sperm for both DNA integrity and functional capacity to complete the post-fertilization process and result in an embryo with the highest likelihood of developing to term. These tests include:

-Sperm DNA Accelerated Decondensation (SDAD) Test

-Sperm DNA Decondensation (SDD) Test

-Sperm Chromatin Fragmentation Assay (SCFA)

Q:

What is the Sperm DNA Accelerated Decondensation (SDAD) Test?

-The SDAD Test is a sperm function test that measures accelerated sperm DNA decondensation rates. Accelerated decondensation indicates reactive oxygen species (ROS) damaged sperm DNA. It identifies infertile males who would benefit from a 3 month Anti-Oxidant Treatment (AOT) BEFORE any assisted reproductive technology (ART) attempts at pregnancy.

Without Anti-Oxidant Treatment (AOT) using a good over-the-counter Male Fertility Supplement (MFS) with anti-oxidants, such individuals will have a low live birth success rate in natural conception or any assisted reproductive technology (ART) attempts at pregnancy including IUI, IVF, and ICSI. By correcting this problem we have found successful, natural conception in many cases, with successful ART attempts at pregnancy consistent with the clinic’s overall IUI, IVF, or ICSI live birth rates.

You can also confirm correction of ROS damage by retesting with the SDAD test after the AOT has been administered for 2-3 months.

Q:

Question: What can cause a delay in DNA decondensation?

There are many environmental and lifestyle factors that can cause problems with sperm. For example, if a man is exposed to harmful chemicals at work, this can certainly cause problems with sperm DNA decondensation. Some additional problematic lifestyle factors include, types of exercise (like bicycling), medicine regimen/history, smoking, drinking, certain hobbies or other industrial factors, or even taking too many hot baths; all can result in damaged sperm. If you have an abnormal SDD Test score, first, you should see a Fertility Specialist.

The good news is that removing yourself from anything that could have caused a delayed decondensation for 3 months and/or the removal of a varicocele, allows for a complete turnover of sperm that in many cases when retested in the MRHP, will have a normal SDD test score. Such individuals have in many cases been able to conceive by natural conception.
